Requirements

  • Child must have been born in the state of Texas.
  • You must be the person named on the certificate, the legal representative, or an immediate family member.
  • Have a valid photo ID such as a Driver License, Federal or State ID card, Military ID, or U.S. Passport. (For other acceptable ID options visit: www.dshs.texas.gov/vital-statistics/acceptable-identification-id)
  • A completed birth certificate application (applications are available in the lobby).
  • Payment of $23 (cash, credit card, or check).
