*Two dental cleanings covered in membership does not include scaling and root planing, which is necessary treatment for patients with periodontal disease.

**Membership covers services received at Pickett Dental Group and Pickett Dentistry at Alden Bridge. Membership benefits are not available for treatment performed at officesto which we refer our patients.
