Memorial Hermann invites community to sign up for vaccine

THE WOODLANDS, TX -- Memorial Hermann invites all members of the community – regardless of vaccination eligibility – to complete its COVID-19 Vaccination Request Form. All members of the community 18 years of age and older can use this online tool to register for the opportunity to receive a COVID-19 vaccine from Memorial Hermann.

Located on the Memorial Hermann website, the online vaccination request form allows individuals to be placed on a waitlist for future appointments at Memorial Hermann’s vaccination clinics. Registered individuals who meet eligibility for vaccination will be notified when appointments are available. Memorial Hermann will make appointments available based on vaccine supply and will prioritize vaccine distribution in accordance with guidance from the Texas Department of State Health Services. At this time, COVID-19 vaccination is open to the state’s Phase 1A, 1B and 1C priority groups, as well as those who work in school and/or childcare operations.
