On a GLP-1? Here's How to Know If You're Losing Fat or Muscle
The scale is down again. That should feel great!
But if you're on semaglutide (Ozempic, Wegovy) or tirzepatide (Mounjaro, Zepbound), you've probably wondered: Is this fat I'm losing, or muscle too?
You're not overthinking it. It's a real question with real stakes. And your bathroom scale can't answer it.
The Short Answer
Most of the weight you lose on a GLP-1 is fat. But not all of it. Studies have consistently found that 25–40% of weight lost on these medications can come from lean mass, meaning muscle and other non-fat tissue.
Some of that is normal. When your body is carrying less weight, it needs less structural support. Researchers call this an "adaptive" response.

Recent research is encouraging. A 2025 study following patients who combined GLP-1 medication with strength training and adequate protein found that some actually gained lean mass while losing significant fat. Another study of 115 patients on semaglutide found that grip strength improved over a year of treatment, even as weight dropped.
The pattern is clear: what you do alongside your medication matters. Three things make the biggest difference.
Strength training. Even basic resistance exercises like squats, push-ups, and rows signal your body to hold onto muscle. You don't need a gym membership. You need to give your muscles a reason to stay.
Protein at most meals. When your appetite is reduced, every bite counts more. Eggs, chicken, fish, Greek yogurt, beans. Including a protein source at meals gives your body the raw materials to preserve muscle.
Tracking what's actually changing. This is the piece most people are missing. If you can see your fat, muscle, and measurements changing over time, you have real information instead of guesses.
